If not, you will be prompted to the next time you come in for or make a new appointment.
To prevent the spread of COVID-19 and reduce the potential risk of exposure to our team and visitors, we are conducting a simple screening questionnaire. Your participation is important and required to help us take precautionary measures to protect you and everyone in this building. Thank you for your time, consideration, and truthful responses.
You agree to reschedule if you cared for someone diagnosed with COVID-19 within the 14 days of your appointment.
You agree to reschedule if you experienced any cold or flu-like symptoms within 14 days of your appointment.
You agree to wear a mask upon entering the building and for the duration of your time spent in the building.
You agree to declare immediately and reschedule your appointment if you have traveled to a COVID hotspot within the last 14 days or knowingly may have been exposed to someone who has symptoms.

Please note that some of our policies have changed in response to COVID-19. As our staff navigates these uncertain waters of changes and policies we appreciate your cooperation and understanding.
Please text your lash artist when you arrive in the parking lot before exiting your car.
Your lash artist will reply to your text with a simple screening questionnaire similar to this form.
Please submit a truthful response and WAIT IN YOUR CAR until you receive a text that it is appropriate to enter the building for your appointment. Our goal is to reduce the amount of people in the building at one time.
Please wear your mask upon entering the building and for the duration of your time in the building.
Please note that no guests are allowed with you at your appointment.
Please head straight to the restroom to wash your hands before entering our studio for your appointment.
FOR YOUR APPOINTMENT
Blankets are still available for use during appointments, however they will be washed after each use. Blankets must be requested.
Leg compression boots are still available for use as well, and will be disinfected after each client.
Please add your credit card to our booking system if you haven't already for a touchless transaction at checkout. If you would like to tip your lash artist, you may ask them to add the amount for you. The ipad will be disinfected after each signature. We will still accept all forms of payment that we previously did.
Upon leaving the studio, please wash your hands again in the restroom.

By replying “I AGREE” to this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by my mere presence within this establishment and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, volunteers, and program participants and their families. I hereby release the booked business from any and all claims arising from or in connection with any direct COVID-19 impact while visiting.
